Restricted Tests
Access and loan of tests in the restricted collection may be borrowed under the following conditions.
1. School of Education faculty members and State of Wisconsin licensed psychologists/psychiatrists on the University of Wisconsin-Madison faculty may request authorization in writing on University letterhead or by e-mail for a specific student or students or staff members to access and/or borrow a specific test or tests. (Faculty members are defined are those individuals whose appointment title is Professor, Associate Professor, Assistant Professor or Instructor, including Adjunct and Clinical appointments at these titles.)
2. Each request for authorization must include the name of the specific test; the name of the specific individual who will be authorized to access and/or borrow the test; the time period during which the authorization may be valid (not to exceed a single academic term or break); the suggested loan period for the test (not to exceed three days); and -if the requestor is not a School of Education faculty member- evidence that the requestor is a licensed psychologist or psychiatrist.
3. Requests for authorization may be submitted to any member of the Circulation Staff of the CIMC. The CIMC staff member must verify that the individual making the request is a School of Education faculty member or University faculty who is a licensed psychologist/psychiatrist. In addition the CIMC staff member must determine that the request for authorization is complete and that the request does not conflict with the previous requests to place specific test titles on restricted reserve. Individuals who wish to borrow the test must have a currently valid University of Wisconsin Madison Staff or Student Identification card.
4. Requests for authorization will be retained by the CIMC Circulation Staff and filed in the Tests section of the Circulation Desk Binder.
5. Test titles will be charged out using the CIMC's Circulation System. Reserve overdue charges will be assessed for any titles which are not returned on time. Test titles may not be renewed.
6. CIMC staff will inventory tests upon return. Users will be charged a replacement fee for the entire test if any part of the test is missing upon return.
7. If an CIMC staff member does not grant authorization to access and/or borrow the test, the requestor may appeal this decision to the Director of the CIMC.
